The flu vaccine that we are offer for you (or your child) has a small amount of thimerosal. At this time, we do not have any flu vaccines without thimerosal.
Thimerosal is a preservative that has mercury in it. Thimerosal helps to keep vaccines free of germs. This preservative has been used in some vaccines and other products since the 1930s.
This vaccine shot is given into the muscle of your arm, buttock, or thigh. You may have local pain, redness or swelling at the vaccination site. There is no convincing proof of harm caused by the low doses of thimerosal in vaccines.
The flu vaccine is the best way to protect against the flu. The failure to get the flu shot places you at risk of getting the flu and complications from the flu, including death. Therefore, the risk of flu is far more than the small risk of a local reaction that may occur with the use of the flu vaccine with a very small amount of thimerosal in it.

Source text - English Loading a Chip
1. Before loading chips, write the chip number on each chip, according to the numbering scheme on the chip worksheet (13 samples are associated with each numbered chip). Samples should be loaded into the corresponding wells of the appropriate chip, according to the worksheet. Refer to the Instructions for Use of Chip Devices for details on running chips.
2. XYZ Software: After placing the chip on the analyzer, select Start.
3. OPQ Software: Type the chip number in the File Prefix field on the Start dialog box.
4. Click Start.
IMPORTANT: The correct chip number must be entered in the File Prefix field. Be careful not to enter spacing either before or after the chip number.
Chip QC
Once chips are run on the analyzer, the chip data automatically exports encrypted data from the analyzer to the ABC Software via a secure Internet connection. Use the following procedure to QC a chip.
IMPORTANT: Before generating reports or building libraries, check all data for quality on the QC screen.
1. Click QC Chip on the Home page, or select Chips & QC > List from the ABC menu bar on the Home page.
2. On the Waiting for QC tab, select the chip name.
In the QC page, two sample graphs for each of the 13 samples will display. The quality of each sample is checked by examining the raw and the normalized graph.
• Raw signal: shows the unprocessed curve with high and low markers.
• Normalized curve: shows the processed curve with the high and low markers removed.
3. If a chip is not listed in the Chips Waiting for QC tab, use the Unlinked Chips tab.

XYZ C-mount couplers are used to attach endoscopes to XYZ surgical camera heads. They facilitate the easy exchange of different sized scopes during a surgical procedure and allow users to focus the image captured through the scope.
Figure 1 labels the parts of the coupler discussed in this manual.
Indications/Contraindications
C-mount couplers are indicated for use in surgical procedures where a XYZ endoscopic camera and endoscope are used. There are no known contraindications.
Setting Up the Coupler
1. Gripping the rear adapter, screw the coupler into the camera head until it forms a tight seal (see Figure 2).
Caution When attaching or removing the coupler, grip only the rear adapter, as twisting other parts of the coupler may result in mechanical damage.

2. Rotate the endobody clockwise (a) until the endobody clamp is aligned with the camera (b) as in Figure 3.
Caution Rotating the endobody counterclockwise may cause the coupler to unscrew from the camera head.

3. Depress the endobody clamp with your thumb and insert a scope into the endobody.
4. Release the endobody clamp to secure the scope.
5. With the camera console turned on, rotate the focusing ring to establish a sharp focus.
6. To remove the coupler, grip the rear adapter and unscrew the coupler from the camera head.

• Do not place pillows, bumpers, fluffy quilts or stuffed toys in the crib. Always keep blankets away from the baby’s crib.
• Side rails should be up at all times, especially as the baby grows.
• Never put your baby to sleep on an adult bed, sofa, waterbed, sheepskin or other soft mattress – even for a nap.
• If you do not have a crib, put your baby to sleep in an enclosed space, such as a playpen or stroller, and on a firm surface with no loose bedding or pillows.
• Don’t let your baby get too warm during sleep. Dress your baby in light sleep clothing, and keep the room at a temperature that is comfortable for an adult.
• Think about using a clean, dry pacifier when placing the infant down to sleep, but don’t force the baby to take it. If you are breastfeeding, wait until your baby is used to breastfeeding before using a pacifier—usually when the baby is about 1 month old.
You do not have to sleep in the same bed to keep your baby close at night:
• Place your baby’s crib or bassinet in your room, near your bed.

MWV is committed to conducting business in an ethical, legal and socially responsible manner. MWV expects its Suppliers to share this same commitment and, as a condition of doing business, requires its Suppliers to follow these Principles of Conduct (“Principles”). Suppliers must take reasonable measures to ensure that any suppliers from which they source act in accordance with these Principles.
Compliance with Laws is Required: MWV Suppliers must operate their businesses in full compliance with all applicable laws, codes, rules and regulations of the countries, provinces, states, regions and municipalities in which they operate.
Child Labor is not Permitted: MWV will not tolerate the exploitation or use of children as workers by Suppliers. MWV shall not contract with any Supplier that employs individuals younger than 15 years of age or the legal minimum age, if higher.
Forced, Prison or Involuntary Labor is not Permitted: MWV will not tolerate the use by Suppliers of forced labor in any form, including slave, prison, indentured or bonded labor.
Discrimination is not Permitted: MWV will not tolerate discrimination by Suppliers of their workers on the basis of race, color, religion, gender, social and ethnic origin, age, marital status, pregnancy, creed or political belief, disability, sexual orientation or any other basis prohibited by law.
Harassment is not Permitted: MWV expects its Suppliers to provide a work environment that is free from any form of harassment, including verbal, physical, or sexual harassment. Threats or acts of retaliation or physical punishment against workers will not be tolerated. At all times, workers shall be treated with respect and dignity.
No Undocumented Workers: Suppliers shall only utilize workers who have a legal right to work.
Compensation (Wages and Benefits): MWV requires that Suppliers compensate employees in compliance with all laws and regulations relating to minimum wages, overtime, piece work, employee benefits, maximum hours worked per day or maximum days worked per week.
Freedom of Association: Suppliers shall recognize and respect the right of workers to associate freely or organize as permitted by applicable laws and regulations and to be free to raise concerns related to employment issues.
Product Quality and Product Safety: All products that Supplier manufactures on behalf of or sells to MWV shall be designed and produced to be safe for their intended uses, and for other reasonably foreseeable uses. MWV is committed to providing products that meet applicable regulatory specifications and quality standards.
Sustainability of Resources: Suppliers must only use materials derived from operations that are in compliance with the environmental and social laws and regulations of the country of origin.
Environment: Suppliers shall meet or exceed all environmental standards required by applicable laws, codes and regulations.
Workplace Safety and Health: Suppliers shall keep the work environment free from recognized hazards and assure compliance with all applicable laws regarding working conditions, including worker health and safety, sanitation, fire safety, risk protection and electrical, mechanical and structural safety.
Security: MWV strongly encourages its Suppliers to implement security controls to secure their international supply chain to comply with anti-terrorism laws in the countries and regions of the world in which they operate and transact business. MWV has specifically partnered with U.S. Customs and Border Protection (CBP) to secure MWV’s international supply chain and so requires all Suppliers of U.S.-bound products to demonstrate that they are meeting Customs-Trade Partnership Against Terrorism (C-TPAT) security criteria, as published by CBP.
Business Practices: Suppliers shall not engage in bribery, kickbacks, collusive bidding, price fixing or other unfair trade practices.
Demonstration of Compliance: Supplier must be able to demonstrate compliance with these Principles at the request and to the satisfaction of MWV. These minimum requirements are part of all agreements between MWV and applicable Suppliers. We expect our Suppliers to develop and implement appropriate internal business processes to ensure compliance with these Principles. MWV routinely utilizes independent third-parties to assess Suppliers’ compliance with these Principles. These assessments generally include confidential interviews with on-site workers. If a Supplier fails to uphold any aspect of the Principles, the Supplier is expected to implement corrective actions. Near Miss Investigation
Near misses need to be investigated. The Manager & Supervisor of the employee involved in the near miss is responsible for conducting the investigation in consultation with Health and Safety Committee and, when appropriate, ensuring that corrective actions are taken with the input of OH&SC. The depth and complexity of the investigation will vary with the circumstances and seriousness of the incident. A thorough investigation may identify previously overlooked physical, environmental, or process hazards, the need for new or more extensive safety training, or unsafe work practices.
The Manager & Supervisor should complete Corrective Actions section on the form, and then forward the form to the Health and Safety Committee for review and approval. Once complete, a copy of the form must be kept as a record.
11. Description of Near Miss: Most accidents result from an accumulation of events. An accurate, factual description of the accident and the events leading up to it can be very helpful. This chronological sequence can be studied to determine how each event may have contributed to the accident. Include photos or drawings of the accident site, if these will be useful to the investigation.
12. Factors: Factors, if any, are the conditions in the workplace or actions that contributed to the occurrence of this accident. Examples might include unguarded machinery, broken tools, slippery floors, not following established procedures, or insufficient training or maintenance.
13. Corrective Actions: List actions or steps that could be taken to control or eliminate the likelihood of a recurrence. Include not only those that can be accomplished right away (e.g., providing personal protective equipment, installing a machine guard), but also actions such as changes in policy or providing additional training. OH&SC should be an integral part of any corrective actions. Please consult the OH&SC prior to implementing corrective actions.

होते-होते टल गई घटनाओं का अन्वेषण करने की जरूरत होती है। होते-होते टल गई घटना में शामिल कर्मचारी का प्रबंधक एवं पर्यवेक्षक स्वास्थ्य और सुरक्षा समिति के परामर्श से इसका अन्वेषण करने और, उपयुक्त होने पर, यह सुनिश्चित करने के लिए जिम्मेदार है कि OH&SC के इनपुट के साथ सुधारात्मक कार्रवाई की जाए। घटना की परिस्थितियों और गंभीरता के आधार पर अन्वेषण की गहराई एवं जटिलता में अंतर होगा। पूर्ण अन्वेषण से पूर्व में नजरंदाज कर दिए गए भौतिक, पर्यावरणीय, या प्रक्रियागत संकटों, नए या अधिक गहन सुरक्षा प्रशिक्षण के लिए आवश्यकता, अथवा कार्य की असुरक्षित प्रथाओं की पहचान हो सकती है।

11. होते-होते टल गई घटना का विवरण: अधिकांश दुर्घटनाएं घटनाओं के संचयन से जन्म लेती हैं। दुर्घटना तथा यहाँ तक पहुंचाने वाली घटनाओं का सटीक, तथ्यपरक विवरण बहुत सहायक हो सकता है। इस बात का निर्धारण करने के लिए इस तिथिवार क्रम का अध्ययन किया जा सकता है कि दुर्घटना में प्रत्येक घटना ने कैसे योगदान दिया होगा। यदि दुर्घटना स्थल के फोटो एवं ड्राइंग अन्वेषण के लिए उपयोगी हों, तो इन्हें शामिल करें।
12. कारक: कारक (यदि कोई हों) कार्यस्थल में ऐसी स्थितियां या कार्रवाइयां हैं, जिन्होंने इस घटना के घटित होने में योगदान दिया। उदाहरणों में अरक्षित मशीनरी, टूटे औजार, फिसलन युक्त फर्श, स्थापित प्रक्रियाओं का पालन न होना, अथवा अपर्याप्त प्रशिक्षण या अनुरक्षण शामिल हो सकते हैं।
