Clinical evolution : Five-year history of episodes of back pain after muscle strain. Subsequent worsening pain irradiation onto S1 at right.
Neurological examination : stiff spine. All flexion-extension movements of the trunk are impaired. Unremitting paraesthesias and pain over the L5 and S1 region. Gait is impaired by constant pain.
Neuroradiological examination : the root of S1 is in continuity with a large cyst at its exit from the root canal.
Operation : release by foraminotomy of periradicular cysts on L5 and S1 at right. The L5 and S1 roots are closer together than normal. The cysts are not completely intradural, as they appear to be arachnoid protrusions at points of dural sac impairment (see post-operative drawing).
Follow-up : one month later, complete disappearance of pain and paraesthesias.
Anatomoclinical considerations : dysembryogenetic cysts along the nerve root course are often multiple and vary in size, and may extend from L1 to the first sacral roots on both sides. We report this case only for morphological precision which highlights the failure to distinguish the primitive perimedullary mesenchyme in the segment in which the relations between motor and sensory roots are established along with the special arrangement of the cell layers of the meninx. The abnormal root covering encountered at surgery was associated with a dilated arterial circulation of the dura mater and abundant epidural lipomatous tissue.

Evoluzione clinica: 5 anni di episodi di lombalgia in seguito a strappo muscolare. Successivo aggravarsi del dolore irradiato in S1 a destra.
Esame neurologico: colonna vertebrale rigida. Tutti i movimenti di flessione ed estensione del tronco sono ridotti. Parestesie e dolore continui al di sopra della regione L5 e S1. Deambulazione compromessa da dolore costante.
Esame neuroradiologico: la radice di S1 è in continuità con una grossa cisti all'uscita del canale radicolare.
Intervento: rimozione di cisti periradicolare in L5 e S1 a destra tramite foraminotomia. Le radici di L5 e S1 sono più vicine del normale. Le cisti non sono totalmente intradurali, dal momento che sembrano essere protrusioni aracnoidee in corrispondenza del punto di danneggiamento del sacco durale (vedi valutazione postoperatoria).
Follow-up: un mese più tardi, totale scomparsa di dolore e di parestesie.
Considerazioni anatomo-cliniche: le cisti disembriogenetiche lungo la radice nervosa sono spesso multiple e di dimensione variabile, e possono estendersi da L1 alle prime radici sacrali in entrambi i lati. Presentiamo questo caso esclusivamente per precisione morfologica, la quale evidenzia l'impossibilità di distinguere il mesenchima perimidollare primitivo nel segmento in cui vengono stabilite le relazioni tra le radici motorie e sensoriali e la particolare disposizione degli strati cellulari della meninge. L'anomalo rivestimento radicolare riscontrato durante l'intervento chirurgico era associato a circolazione arteriosa dilatata della dura madre e ad abbondante tessuto lipomatoso epidurale.

This NON-CIRCUMVENTION/NON-DISCLOSURE AGREEMENT is between ..............................Limited of………, and the Client of (see organization’s address) and is effective as of the date affixed to the signatures on the second and last page hereof.
............................... Limited and the Client, and their respective officers, representatives, agents, consultants and affiliated companies, constitute the PARTIES to this AGREEMENT, as outlined in the following paragraphs.

Limited and the Client agree to exchange and discuss certain confidential information, documentation, plans, company materials and other, related to projects, clients, financing contacts or business, in order to evaluate and implement the structure of the client or a possible financing transaction, acquisition, investment, joint venture, merger, consolidation, service engagement, or other business activity.
1. Both PARTIES shall keep all information exchanged or discussed confidential for a period of five (5) years from the date of its disclosure. During this period, information shall not be disclosed to any third parties, without prior written consent of either party, except as authorized herein.
2. Dissemination of Confidential Information, by either of the PARTIES, shall be limited to their employees, consultants, agents, clients and financial schemes, etc., whose duties require them to use the information, on the basis of a clear understanding by such persons of their obligations to maintain the confidentiality of the information and to restrict its use. Both PARTIES shall properly notify each other of any such dissemination of Confidential Information.
3. Both PARTIES shall abide by the spirit of non-interference and non-circumvention regarding business relationships established by and/or introduced by either party to the other. Both PARTIES agree to refrain from circumventing the other party in any dealings with clients, persons, or organizations introduced by the other party, either in person or by fax, telephone or mail.
