Systemic features are common, with weight loss, fever, or fatigue often present.

Os sinais e sintomas de espondilite anquilosante frequentemente surgem gradualmente, com maior incidência entre 20 e 30 anos de idade. Os sintomas iniciais são normalmente dor surda crônica na região lombar ou glúteos combinada com enrijecimento da região inferior das costas. Indivíduos frequentemente sofrem dor e rigidez que os acordam nas primeiras horas da manhã. Com o progresso da doença, são vistas perda da mobilidade espinhal e expansão do tórax, com limitação de flexão anterior, lateral e extensão da espinha lombar.
