Dear Parent, Guardian, or Caretaker:
I am a graduate student from the University of Southern California currently working in your child’s classroom.
During my time in the classroom, I will need to complete several assignments that will require me to video record my teaching of lessons to your son or daughter’s class, and at times collect samples of your child’s work.
I am writing to request your permission to allow me to video record and collect student work samples from your child. The recordings and work samples will be seen by my professors, by program staff, and by my peers in my classes. The focus of my filming will be on teaching and student learning in the class.
Your child’s grades, standing, and treatment will not be affected by your decision to allow your child to be filmed and submit work samples.

እኔ በአሁኑ ጊዜ በልጅዎ ክፍል ውስጥ የምሠራ ከደቡብ ካሊፎርኒያ ዩኒቨርሲቲ ክፍተኛ ደረጃ ተማሪ ነኝ።
