(…) Il a dit : “Je veux comprendre les utilisations et les implications de cette formidable technologie.” Je me suis réveillée, couverte d’une sueur froide. » Jennifer Doudna a raconté, en novembre 2015, ce cauchemar au New Yorker, qui enquêtait sur CrisprCas9, un puissant outil d’édition du génome que la chercheuse à l’université Berkeley a contribué à mettre au point. Un dictateur pourraitil aujourd’hui ressusciter les délires eugénistes des nazis, produire des lignées de « bébés parfaits » grâce à ces nouveaux outils ? Le « meilleur des mondes » estil à notre porte ? Cette perspective est suffisamment inquiétante pour que Jennifer Doudna et ses pairs, mais aussi de nombreuses sociétés savantes – et même la CIA –, se soient emparés du brûlant sujet CrisprCas9, sur son versant éthique. Jamais l’humanité n’a semblé aussi proche de modifier sa propre lignée, son génome et celui des générations à venir. Il ne s’agirait plus de sciencefiction, d’un scénario dystopique, mais d’une possibilité qui a émergé avec force en avril 2015 : une équipe chinoise publie alors les résultats d’une expérience sur des embryons humains visant à modifier le gène responsable de la bêtathalassémie, une forme d’anémie d’origine génétique. De plus, leur étude suggère que CrisprCas9 n’est pas l’outil à la précision chirurgicale tant vantée : seul un faible nombre d’embryons modifiés porte les mutations souhaitées, et des modifications « hors cibles » ont été mises en évidence. Même constat un an plus tard, lors de la parution d’une nouvelle étude chinoise où CrisprCas9 est cette fois utilisé pour offrir une protection contre le VIH : le succès n’a été que partiel, avec de nombreuses mutations non voulues et des ratés dans l’édition des embryons, détruits après quel
ques divisions cellulaires. Les chercheurs chinois voulaient voir si l’on pourrait créer des humains naturellement immunisés contre le sida. Ils ont prouvé qu’on était loin du compte. Le bébé « sur mesure » n’est finalement pas pour demain. Dans l’intervalle, une équipe de l’Institut FrancisCrick, à Londres, a reçu l’autorisation de procéder, elle aussi, à des manipulations sur des embryons humains. Il s’agit de désactiver de façon sélective certains gènes considérés comme cruciaux dans la différenciation des premières cellules en divers tissus. A Stockholm aussi, une équipe pourra procéder à de tels essais. Comme en Chine, pas question d’implanter ces embryons dans un utérus. Le but est de mieux comprendre certaines formes d’infertilité. Ces expérimentations sur l’embryon sont conformes au consensus qui a émergé au fil des réunions internationales et des réflexions conduites par les sociétés savantes nationales, de l’usage sur l’homme des nouvelles techniques d’édition du génome. « Crispr fonctionne si bien et rencontre un tel succès qu’il serait important d’évaluer les aspects éthiques de son utilisation », avait prévenu, dès juin 2014, la Française Emmanuelle Charpentier, coinventrice de l’outil. Au printemps 2015, Nature et Science publient des mises en garde contre la modification des cellules germinales (sexuelles) qui passerait d’une génération à l’autre. Une de ces tribunes est cosignée par le Nobel de chimie, en 1980, Paul Berg. Ce dernier avait organisé, en 1975, la conférence d’Asilomar (Californie), qui avait abouti à la mise en place de protections contre les fuites dans l’environnement des premières bactéries génétiquement modifiées. Mais, cette fois, il s’agit de changer le patrimoine héréditaire de la lignée humaine ellemême. Jusqu’où peuton aller ? Une réunion internationale est organisée, début décembre 2015, à Washington. Après des débats enflammés, la déclaration finale juge que la recherche fondamentale et préclinique sur l’édition des gènes est nécessaire et doit être poursuivie, ainsi que sur les bénéfices et risques potentiels de leur usage clinique. Mais, « si, dans ce processus de recherche, des embryons humains et des cellules germinales subissent des éditions de gènes, les cellules
modifiées ne devront pas être utilisées pour lancer une grossesse », préviennent les organisateurs. L’usage clinique de ces techniques sur les cellules somatiques (non transmises d’une génération à l’autre) doit s’inscrire dans les dispositifs « existants et évolutifs » qui encadrent les thérapies géniques. Cette position est rejointe peu ou prou par diverses sociétés savantes et organismes de recherche, avec des nuances selon les législations nationales. La France, comme la plupart des pays d’Europe, est signataire de la convention d’Oviedo (1997), dont l’article 13 stipule qu’« une intervention ayant pour objet de modifier le génome humain ne peut être entreprise que pour des raisons préventives, diagnostiques ou thérapeutiques, et seulement si elle n’a pas pour but d’introduire une modification dans le génome de la descendance ». Une interprétation maximaliste du texte pourrait interdire toute utilisation de Crispr sur les cellules germinales. Mais le neurobiologiste Hervé Chneiweiss, président du comité d’éthique de l’Inserm, n’en fait pas la même lecture : « Il faut établir une distinction entre la recherche fondamentale autorisée et le transfert vers les applications humaines. » Audelà, quand la technologie sera éprouvée, s’interrogetil, « en quoi cela seraitil une atteinte à l’humanité d’éradiquer des maladies d’une particulière gravité, comme celle de Huntington, en modifiant les embryons ? » George Church (Harvard) ne se satisfait pas du consensus actuel. Pour lui, la focalisation sur l’embryon a fait passer au second plan l’édition des cellules sexuelles masculines : « En partant de cellules souches, vous pouvez les modifier ex vivo, en faire des clones, et vérifier celles qui ont les bonnes modifications. On peut s’assurer qu’elles sont parfaites. » Et les utiliser pour éviter d’éliminer des embryons. Pour Alain Fischer (ImagineNecker), « père » des bébésbulles soignés par thérapie génique, cette vision relève de la « sciencefiction délirante ». Crispr constitue un outil de recherche « incontournable » et prometteur pour les cellules somatiques (adultes), mais modifier les cellules germinales revient in fine « à toucher au patrimoine de l’humanité, ce qui n’est pas raisonnable et doit rester interdit ». Le biologiste de la reproduction Pierre Jouannet, qui a corédigé plusieurs rapports de sociétés
savantes françaises sur Crispr, estime que George Church a raison d’insister sur le potentiel des cellules germinales, même s’« il ne faut pas être naïf » et que les défis à relever sont immenses. Ils le sont aussi pour les thérapies géniques imaginées sur les cellules adultes, moins problématiques d’un point de vue éthique. Là encore, la « magie Crispr » se heurte à des obstacles parfois sousestimés, comme les mutations hors cibles. Keith Joung, du Massachusetts General Hospital, a mis les pieds dans le plat, début juillet, devant la Société américaine d’hématologie, en projetant une diapositive montrant un individu la tête dans le sable. Comme le raconte la revue en ligne Stat, il a souligné les carences des logiciels utilisés pour déterminer les zones du génome susceptibles d’être modifiées par inadvertance par Crispr – ce qui a douché l’enthousiasme général. L’autre grand défi, c’est la faculté de faire s’exprimer les cellules mutées par Crispr dans les
bons tissus. La société Editas cible par exemple des maladies de l’œil, un organe qui se prête à l’injection de virus vecteurs de Crispr. Crispr Therapeutics mise sur une stratégie assez similaire. Intellia Therapeutics parie sur des nanoparticules lipidiques pour transporter Crispr jusqu’au foie, où il permettrait de lutter contre diverses maladies comme l’hémophilie. D’autres, comme David Bikard à l’Institut Pasteur, espèrent retourner Crispr contre les bactéries qui l’ont inventé, pour lutter contre les souches résistantes aux antibiotiques – là encore la question du vecteur sera essentielle. La pédiatre Marina Cavazzana (Imagine Necker), qui a vécu les hauts et les bas de la thérapie génique des bébésbulles, est très enthousiaste sur le potentiel de Crispr. « Je suis amenée à relire les résultats précliniques d’autres groupes dans le monde, encore non publiés, qui sont très impressionnants », ditelle. Mais, de l’animal à l’homme, les embûches peuvent être nombreuses, prévientelle : « Les chercheurs ne perçoivent pas toujours que l’application clinique est un très long chemin. » Les startup pionnières, basées à Boston, espéraient être les premières à passer à ces essais cliniques. Elles viennent de se faire dépasser par une équipe de l’université de Pennsylvanie, qui a reçu, fin juin, un feu vert des Instituts nationaux de la santé (NIH) américains pour tester une thérapie ex vivo qui viserait simultanément trois gènes. L’idée reprend avec Crispr la stratégie dite des cellules CART déjà mise en œuvre avec succès avec des outils plus anciens d’édition des gènes. L’un d’eux, développé par la société française Cellectis, a permis de sauver une petite Londonienne d’une leucémie, fin 2015. Cette thérapie consistera à prélever des lymphocytes T, des cellules immunitaires, et de les modifier pour qu’elles s’attaquent à des cellules tumorales une fois réinjectées à des patients souffrant de mélanome, sarcome ou myélome résistants aux traitements classiques. Certains se sont émus que la Penn State se retrouve aux avantpostes. En 1999, Jesse Gelsinger, un jeune homme de 18 ans, était mort lors d’un essai clinique de thérapie génique conduit dans cette université. On avait ensuite découvert que le directeur de l’étude, James Wilson, possédait des parts dans Genovo, une compagnie qui avait un intérêt direct à hâter sa réussite. Et que le patient, qui n’avait pas été correctement informé des risques, avait reçu des doses plus fortes que prévu. Carl June, le conseiller scientifique de la nouvelle étude, possède des brevets sur la technologie testée, mais il assure que des mesures seront prises pour surmonter ce conflit d’intérêts. L’essai sera financé par un institut créé, en avril, par le milliardaire Sean Parker, cofondateur de Napster et associé de Facebook, qui a injecté 250 millions de dollars (223 millions d’euros) dans un vaste programme d’immunothérapie. Cet ancien petit génie de l’informatique, âgé de 36 ans, s’est mis en tête de « hacker le cancer ». Mais, encore une fois, la Chine double tout le monde : le 6 juillet, une équipe de l’université du Sichuan a reçu l’autorisation de procéder à un essai clinique du même type, ciblant le cancer du poumon. This perspective is sufficiently worrying to make Jennifer and her peers as well as numerous groups of scientists - even the CIA – seize the burning subject of the ethics of Crispr-Cas9. Never before had humanity seemed closer to modifying its own breed, its genome and that of the generations to come.
It is no longer a question of science fiction or a scenario of dystopian but of a possibility that emerged strongly in April 2015 when a Chinese team published the results of an experiment on human embryos aiming to modify the gene responsible for beta-thalassemia, a form of anaemia of genetic origin. The study caused a big fuss Some people thought that the obstacle that ethics built up was insurmountable. The Chinese researchers thought that they had taken appropriate precautions for they had only used triploid cells that were not able to develop into a
viable being – they stopped observing when the embryos had just eight cells.
What is more, their study suggested the Crispr-Cas9 is not a tool that has the much boasted surgical precision for only a small number of modified embryos carry the desired mutations and ‘off target’ modifications showed up. The same thing was noted a year later when a new Chinese study appeared where Crispr-Cas9 was used to offer protection, this time against HIV but success was only partial with numerous unwanted mutations and failures among embryo edition, destroyed after a few cellular divisions. The Chinese researchers wanted to see if human beings who were naturally immune to AIDS could be created. They had proved that we had a long way to go. The designer baby is in the end not a being for tomorrow.
In the meantime, a team from the Francis-Crick Institute at London also received authorisation to proceed with manipulations on human embryos. It is a question of deactivating certain genes in a selective way, genes that are considered crucial to the differentiation of the first cells in various tissues. At Stockholm also, a team will be able to go on to such tests. Just as in China, implanting these embryos into a uterus is out of the question. The goal is to gain better understanding of certain forms of infertility.
These experiments on the embryo are in accordance with the consensus that merged during the course of international meetings and the thought put in by national groups of scientists on the use of new genome editing techniques on Man. ‘’ Crispr functions so well and is so successful that it is important to assess the ethics aspects of its use’, warned the French Emmanuelle Charpentier, co-inventor of the tool, in June 2014.
In spring 2015, Nature and Science published precautions to be taken when manipulating germ cells that pass from one generation to another. One of these forums was co-signed by the Nobel prize winner for chemistry in 1980, Paul Berg. He had organised the Asilomar conference (California) in 1975 that resulted in the enforcement of measures that protect against escape of the first genetically modified bacteria into the environment.
However, this time it was a question of changing the inheritance of the human breed itself. How far can we go? An international meeting was organised, beginning in December 2015, at Washington. After heated discussions, the final declaration was that fundamental and pre-clinical research on the editing of genes was necessary and should be pursued as well as finding out the potential benefits and risks of their clinical use. However, ‘if, during the research process, human embryos and germ cells undergo gene editing, the modified cells must not be used to begin a pregnancy’, warned the organisers. The clinical use of these techniques on somatic cells (that cannot be transferred from one generation to another) must be registered in the ‘existing and developing’ mechanisms that surround genetic therapies.
This position is also taken to a greater or lesser extent, by various groups of scientists and research organisations, with qualifications from national legislation. France, like most European countries, also signed the Oviedo Convention (1997) of which article 13 stipulates that ‘a procedure that aims to modify the human genome can only be undertaken for preventative, diagnostic or therapeutic reasons and only then if the aim is to introduce a genetic modification in the genome of the descendants’. One maximalist interpretation of the text could forbid all use of Crispr on germ cells.
However, the neurobiologist, Hervé Chneiweiss, president of the Inserm Ethics Committee does not agree with this reading. ‘We must establish a distinction between authorised, fundamental research and transfer to human applications.’ Beyond this, when technology is approved, he asked himself, ‘In what respect will this be an attack on humanity that eradicates illnesses of a specific severity, such as Huntington’s, by modifying embryos?’
George Church (Harvard) is not satisfied by the present consensus. For him, focussing on the embryo puts the editing of masculine sex cells in second place. ‘Starting with stem cells, you can modify them in vitro, make clones from them and examine the ones that have good modifications. We can make sure that they are perfect’. And use them to avoid elimination of embryos.
For Alain Fischer (Imagine-Necker), ‘father’ of the boys-in-the-bubbles cured with genetic therapy. This vision is a product of the ‘delirium of science fiction’. Crispr is a ‘vital’. research tool that promotes somatic (adult) cells but modifying the germ cells ultimately harks back to ‘affecting the legacy of humanity and this is not reasonable and must remain forbidden’. The reproduction biologist, Pierre Jouannet, who has co-directed several reports on Crispr made by groups of scientists, reckons that George Church is right to insist on the potential of germ cells even if ‘we should not be naïve’ and if the challenges to be raised are immense.
They are also huge for the genetic therapies imagined for adult cells. These are less problematic from the point of view of ethics. There again, the ‘magic Crispr’ smashes against obstacles that are sometimes underestimated, such as mutations that are outside targets. Keith Joung, from Massachusetts General Hospital, got himself into trouble at the beginning of July with the American Society of Hematology by giving a slide show that showed an individual with his head in the sand. Just as the on-line review Stat recounts, he emphasised the deficiencies of the software used to determine the areas of the genome likely to be modified in advertently by Crispr – that dampened down enthusiasm in general.
The other great challenge is the ability to make silenced cells express using Crispr in good tissues. The company Editas, for example, targets illnesses of the eye, an organ that lends itself to injection of Crispr virus vectors. Crispr Therapeutics is interested in a quite similar strategy and Intellia Therapeutics bets on lipid nanoparticles to transport Crispr to the liver where it will be possible to fight against various illnesses such as hemophilia. Other people, like David Bikard at the Pasteur Institute, hope to turn Crispr against the bacteria that invented it to fight against cultures resistant to antibiotics – there again it is a question of the vector that will be essential.
The pediatrician, Marina Cavazzana (Imagine-Necker), who lived through the ups and downs of genetic therapy for the boys-in-the-bubbles, is very enthusiastic about the potential of Crispr. ‘I am led to re-read the preclinical results of other groups in the world that are still not published but which are very impressive’, she said. ‘However, the pitfalls from animal to Man can be numerous’, she warned. ‘The researchers do not always realize that the clinical application is a very long route.’
Start-up pioneers based in Boston, were hoping to be first to go on to clinical trials. They have just managed to overtake a team from the University of Pennsylvania and at the end of June they received the go-ahead from the American National Institutes of Health (NIH) to test ex vivo therapy that simultaneously targeted three genes. The idea takes up again with Crispr, the strategy said to be from CAR-T cells that has already been made to work with success with the oldest gene editing tools. One of them, developed from the French company Cellectis, made it possible to save a small woman from London from leukaemia at the end of 2015. This therapy is to consist of taking samples of t lymphocytes, immunity cells and modifying them to make them attack tumour cells once injected into patients suffering from melanoma, sarcoma or myeloma that are resistant to classic treatment.
Certain people are moved because the Penn State finds itself again among the leaders. In 1999, Jesse Gelsinger, a young man of eighteen years of age died in a clinical trial of genetic therapy conducted at this university. Afterwards, it was discovered that the director of the study, James Wilson, owned shares in Genovo, a company that had a direct interest in speeding up his success. Also the patient, who hadn’t been correctly informed of the risks, had received higher doses than planned. Carl June, the scientific adviser for the new study, had patents for the technology tested but he confirmed that measures would be taken to overcome this conflict of interests.
The trial was to be financed by the institute created, in April, by the millionaire Sean Parker, co-founder of Napster and associate of Facebook who injected 250 million dollars into a vast programme on immunotherapy. This ex-little IT genius aged 36 years, took it into his head to ‘hack into cancer’.
However, once again, China overtook the whole world because on 6 July, a team from the University of Sichuan received authorisation to proceed with a clinical trial of the same type, targeting cancer of the lung. It could begin as early as the month of August.
If the tycoons from Silicon Valley and the China get together, who knows how far Crispr will go? Invented by bacteria, thousands of years ago to defend against viruses, transformed into a revolutionary tool for editing genes by researchers and from horizons as diverse as the study of yogurt, the plague and cell structures, often motivated out of pure curiosity but ready to do battle over patents and the Nobel prize. It is also a great revealer and its story is that of the science of today.

My education is good including Chemistry and Zoology 'A'-level courses as well as English and French ones. My degree is in English language and literature and in modern French language and culture. Postgraduate diploma modules included critique of others' translations, linguisitics, translation both ways, using the language pair English and French and computer-assisted translation.
Usually I translate from French into English. I've done translation of newspaper and magazine articles, literature and other general texts but mainly scientific and medical translations. I have a published book of biological research translations and would like to do another scientific book. I also work on official documents such a certificates and academic transcripts.
I have also translated two plays for the Bristol Old Vic Theatre School but I particularly welcome medical and scientific research.
I work from home and have all the necessary equipment, PC, scanner, a telephone and email address. I have many reference books,including modern dictionaries, and French contacts.
