Source text - English Abc hereby certifies that he has not received Medicare benefits for any medical treatment, including treatment for any work-related injuries as a result of the September 01, 2010 accident at issue in this amount, and does not currently have pending an application for Medicare benefits.
Abc certifies that the purpose of this compromise settlement is not to shift liability for payment of medical expenses resulting from the September 01, 2010 accident to Medicare. Abc understands that Insurance Company have relied upon this certification in consideration of and for this settlement of Abc’s claim.
Approval and payment of this settlement will discharge Employer of the liability for the payment of any future compensation and medical benefits under the Act arising out of the September 01, 2010 accident sustained by Abc.
