disagree Natalie Pavey (X) : Unless this translation is specifically about Mali, I would not use the source term. Additionally, I do not believe it is good practice to leave words untranslated unless it is the name of an organization that does not function in the target language.

communes and community circles

If this translation concerns West Africa in particular, where I lived for two years, I believe that commune directly translates as it is a local form of government (see my link below). I also believe that "cercles" directly translates in that it refers to discussion circles which are a common practice in West Africa. I would offer the translation: "... transferred from the State to local collectivities via communes and community circles as they pertain to health care."
