Since we are talking about the dental plan, it could very well be the claim rate. In isolation, it can also mean the accident rate.

: In the insurance context, the Portuguese term 'sinistro' is 'claim'. We also have dexter / sinister for right / left. And with Halloween approaching there is the much more common use of sinister in English :o)

One could say increase in the loss ratio, but "worsening loss experience" is how it would normally be expressed in UK insurance circles.
