Not sure the exact term to be used.
I suppose it is an insurance policy plan which requires addional premium (เบี้ยประกัน) from an employee. If the employee wants to increase the coverage, it would cost him to make the difference.

There may be a case that contribution is from the company or others???. Please seek another comment for confirmation. - May L.
