These two terms appear several times in the document.

requerimiento de instrucción formal: information (Definition) Prosecutorial charging document > Ton West III's ES/AmE dictionary.

I think that means laying of a criminal information, namely charge, but seems one stage earlier in letters rogatory for taking of evidence.

request for (judicial) assistance [both terms]

Hard to tell whether "investigation" is being asked for - it might simply be interviewing wtnesses, hence I have used a neutral term, albeit one used in letters rogatory.

request for a report

(Slightly stronger than "request for a reply".)
If the author wanted to say "report", he/she could well have used "informe", but legal documents are often quite formalistic and rigid in their choice of words.
Perhaps it means "a report or other reply".
