@Lisa Remember it's a letter and there are many people who find it difficult to express themselves in writing, and have absolutely no experience writing documents that are going to be used in a Court of Law.

It seems as though the mother wants (it is in her interest) her son/daughter to go abroad (possibly for longer than just a holiday) because she's too ill to look after the minor herself.

No Sergio I'm not guessing, it's what the text says. My translation isn't in "legal speak" as I'm not a specialist in this field, so I haven't posted it as an answer, just as a pointer.

The sentence is badly written As I am not well versed in legal matters, here my translation:
I, XXX, grant my YYY permission to take abroad my zzz for reasons of health and to represent my interests in this matter.

xxx Unless the text is quite distorted, xxx cannot refer to the name of the mother of a minor, since it's preceded by "mi".

I think there's something wrong with the Spanish, perhaps a word missing after "como". From the context, the intended meaning is about certainly "act on my behalf".
What does the second XXX represent, just out of interest?
