This is part of a letter giving power of attorney to a baby's mother to register his birth and request citizenship from abroad. The final part of the sentence is:

...e requerer a inscrição e registro de nascimento atributivo da nacionalidade portuguese de XXX XXX ... podendo requerer, se for o casso disso, a manutenção da composição originária do nome, practicando, requerendo e assinando o que for necessário aos indicados fins.


Could it be a reference to the fact that in this case the name given to the baby is British style, AKA first name + surname, so the writer is suggesting he may need additional surnames mentioned or added as per Portuguese tradition?

I believe the author is referring to how the name is structured/formed. Apparently, Portugal has specific rules in that regard. See https://irn.justica.gov.pt/Servicos/Cidadao/Nascimento/Compo... I would simply translate as “keep the original name” or perhaps “keep the original name structure”. I prefer the former.
