Background and aims: Patients with relapsing-remitting multiple sclerosis (MS) who received 2 years’ treatment with Cladribine Tablets (CT) 10 mg, (cumulative dose 3.5 mg/kg [CT3.5]) in CLARITY followed by 2 years’ placebo (PBO) in CLARITY Extension (CP3.5) experienced durable clinical benefits. The rate and severity of relapse (proxy indicators; hospitalisation and steroid use) were evaluated.
Methods: Qualifying relapse was defined by Kurtzke Functional Score status and specified clinical parameters. Unadjusted annualised relapse rates (ARRs; Week 96) were calculated for CP3.5 patients (N=98). ARRs were also calculated for qualifying and all relapses requiring hospitalisation or steroid treatment. All analyses were posthoc and descriptive.
Results: ARRs (CLARITY; CT3.5 and PBO, CLARITY Extension; CP3.5) for qualifying and all relapses (qualifying and non-qualifying) at Week 96 and those leading to hospitalisation or requiring steroid treatment are presented in Table 1. In CLARITY (CT3.5; N=433), ARR for qualifying relapses was 0.15; and for PBO (N=437) was 0.35. CLARITY Extension: ARR for qualifying relapse in CP3.5 (N=98) at Week 96 was 0.14. In CLARITY CT3.5 and in CLARITY Extension CP3.5, ARR for all relapses, as well as those leading to hospitalisation or requiring steroid treatment were low, relative to CLARITY PBO.
Conclusion: In CLARITY, CT3.5 was efficacious against relapse versus PBO regardless of relapse type. Relapse frequencies at Week 96 in CLARITY Extension were similar to those in CLARITY CT3.5 (no formal comparison).
Relapse in CLARITY Extension CP3.5 were notably lower than for CLARITY PBO. Therefore, the efficacy demonstrated in CLARITY was sustained for each relapse type in CLARITY Extension without further treatment.

Abstract
Despite favorable DNA transfer efficiency, gene expression from recombinant adeno-associated virus (rAAV2) vectors in the lung has been variable in the context of cystic fibrosis (CF) gene therapy. This is due, in part, to the large size of the CF transmembrane regulator (CFTR)-coding sequence which necessitates the use of compact endogenous promoter elements versus stronger exogenous promoters. We evaluated the possibility that gene expression from rAAV could be improved by using AAV capsid serotypes with greater tropism for the apical surface of airway cells (i.e. rAAV5 or rAAV1) and/or using strong promoters such as the cytomegalovirus (CMV) enhancer/chicken beta-actin hybrid (Cβ) promoter. The relative activity of the CMV immediate-early (CMVie) promoter, the Cβ promoter, and the Cβ promoter with a downstream woodchuck hepatitis virus post-transcriptional regulatory element (wpre) were assessed in vitro and in vivo in C57Bl6 mice using human alpha-1 antitrypsin (hAAT) as a secreted reporter. In vivo, the Cβ-AAT-wpre group achieved maximum serum levels of 1.5 mg/ml of hAAT. AAV capsid serotypes were then compared in vivo utilizing the transcriptionally optimized CB-wpre cassette in rAAV serotype 1, 2 or 5 capsids (rAAV1, rAAV2, and rAAV5), utilizing luciferase as a reporter to compare expression over a wide dynamic range. The pulmonary luciferase levels at 8 weeks were similar in rAAV5 and rAAV1 groups (2.9 ׳ 106 relative light units (RLU)/g tissue and 2.7 ׳ 106 RLU/g tissue, respectively), both of which were much higher than rAAV2. Although the advantage of rAAV5 over rAAV2 in the lung has already been described, the availability of another serotype (rAAV1) capable of efficient gene transfer in the lung could be useful.

This guideline addresses only those impurities in new drug products classified as degradation products of the drug substance or reaction products of the drug substance with an excipient and/or immediate container closure system (collectively referred to as “degradation products” in this guideline). Generally, impurities present in the new drug substance need not be monitored or specified in the new drug product unless they are also degradation products (see ICH Q6A guideline on specifications).
Impurities arising from excipients present in the new drug product or extracted or leached from the container closure system are not covered by this guideline. This guideline also does not apply to new drug products used during the clinical research stages of development. The following types of products are not covered in this guideline: biological/biotechnological products, peptides, oligonucleotides, radiopharmaceuticals, fermentation products and semi-synthetic products derived therefrom, herbal products, and crude products of animal or plant origin. Anorexia nervosa
A. Refusal to maintain body weight over a minimally normal weight for age and height (e.g. weight loss leading to maintenance of body weight less than 85% of that expected), or failure to make expected weight gain during period of growth, leading to body weight less than 85% of the expected.
B. Intense fear of gaining weight or becoming fat, even though underweight.
C. C. Disturbance in the way in which one’s body weight or shape is experienced, undue influence of body shape and weight on self-evaluation, or denial of the seriousness of current low body weight. In postmenarcheal women, amenorrhea, i.e. the absence of at least three consecutive menstrual cycles (a woman is considered to have amenorrhea if her periods occur only after hormone, e.g. estrogen, administration).
Restricting type: During the current episode of anorexia nervosa, the person does not regularly engage in binge eating or purging behavior (i.e. self-induced vomiting or the misuse of laxatives, diuretics or enemas).
Binge eating/purging type: During the current episode of anorexia
nervosa, the person has regularly engaged in binge eating
or purging behavior (i.e. self-induced vomiting or the misuse

Bulimia nervosa
A. Recurrent episodes of binge eating. An episode of binge eating is characterized by both of the following:
(1) Eating, in a discrete period of time (e.g. within any 2-h period), an amount of food that is definitely larger than most people would eat during a similar period of time and under similar circumstances.
(2) A sense of lack of control over eating during the episode (e.g. a feeling that one cannot stop eating or control what or how much one is eating).
B. Recurrent inappropriate compensatory behavior in order to prevent weight gain, such as self-induced vomiting; misuse of laxatives, diuretics, or other medications, fasting, or excessive exercise.
C. The binge eating and inappropriate compensatory behaviors both occur, on average, at least twice a week for 3 months.
D. Self-evaluation is unduly influenced by body shape and weight.
E. The disturbance does not occur exclusively during episodes of anorexia nervosa.
Purging type: The person regularly engages in self-induced vomiting or the misuse of laxatives or diuretics.
Non-purging type: The person uses other inappropriate compensatory behaviors, such as fasting or excessive exercise, but does not regularly engage in self-induced vomiting or the misuse of laxatives or diuretics.
