I think the point here with "no congruente" is to call attention to that which is already implied by the "hipertímico": that the affect is inappropriate, the happiness the patient feels doesn't make sense in the context of the real-world situation the patient is in.

What is Inconguence?
When you were a little boy or girl, did you want to grow up to be a firefighter, a movie star or a ballet dancer? At some point you probably gave up that dream in favor of something more attainable (unless you currently are a firefighter, movie star or ballet dancer). When our dreams appear to be out of reach, the result is often frustration, stress and anxiety. Many people wind up abandoning their dreams, believing they are impossible to achieve. These individuals have learned the pain of incongruence.

Carl Rogers introduced the concept of incongruence to psychology in the 1950s. Although general use of the word has come to mean inconsistent or incompatible, Rogers had a more specific definition in mind. He defined congruence as the matching of experience and awareness. Incongruence was therefore lacking congruence, or having feelings not aligned with your actions.

When someone is in a state of incongruence they are said to be having a total opposite experience. This is meaning that the experience they are having is unacceptable to them and is distorted in there self- image. Incongruence is a difference between an actual experience and the self- image of the individual as what they seem to be represented by its experience.
Although we want to see ourselves in a way that is that is similar to our self- image, we sometimes use defense mechanisms such as denial and/ or repression. This is used in order to feel less of a threat by what we would consider unwanted and unpleasant feelings. A person who has a self- concept that is incongruent with there real feelings and experiences will stand up for themselves because the truth really does hurt.

Mood incongruence is a term used to describe a serious symptom of bipolar disorder. It is a psychotic feature of the disease wherein the person's belief or action, whether by hallucination or delusion, does not match with his or her mood.

By contrast, mood congruence also describes a psychotic symptom of bipolar disorder, but, in this case, the belief or action are consistent with that person's mood.

While the difference between mood congruence and incongruence may seem of little consequence — given that they both related to a psychotic episode — the way in which each impacts a person's ability to function and thrive can be strikingly different.

Examples of Mood-Incongruent and Mood-Congruent Symptoms
Incongruent means "conflicting." As such, mood incongruence implies that the symptoms conflict with the person's current mood. Examples include:

Laughing when your dog dies
Believing you have super powers despite going through a major depressive episode
In each case, the actions of the person do not match either the situation or emotional state. The delusion of super powers, for example, in no way coincides with themes of powerlessness that are inherent in depression.

By contrast, congruent means "in agreement." In this case, any symptoms, however extreme, are considered mood-congruent when they in agreement with the person's current mood.
