I don't think you'd issue a certificate just to say that the previous one was wrong. You'd just produce a new one to replace it.

So I think 'correction of error' is the right answer, and you should post it.

The form is linked to from this page, which says 'correction' and seems more likely than 'certificate'.

But it's also hard to argue with Jean-Marie's reference - I wonder if 'certificate' is a misreading of 'COE'.
