Arabic to English Medical Medical (general) consent form

I am concerned about using "by informed consent" as موافقة عن وعيhas been used for this all the way through the document, any ideas?

The confusion arises from the absense of visual means to set off the object of the verb "to read." The phrase should read

قادر على قراءة " استمارة الموافقة عن فهم وإدراك" الخ الخ الخ

In other words, الموافقة عن فهم وإدراك is merely a modifier of the noun "form."

"by informed consent"

The full sentence would be helpful, but yes: literally, a kind of literary way of saying something like "by (his own) understanding, having had it explained to him."
