The term in French is very specific and with a legal connotation. See reference below.
In English, the current terminology is "health care proxy". It is much more than the non-specific word meaning of a person who has the trust of the patient.

In the UK, people may appoint a health care proxy under the Mental Capacity Act 2005, but it is referred to as a lasting power of attorney. [8]

: NB :I dont know who the translation is for, but there are differences between the US and UK medico-legal systems regarding 'consent' for clinical trials on people who cannot make decisions for themselves-England and Scotland NHS are different too!!

Discussion
There may also be other terms to the standard "health care proxy" such as legal representative in some jurisdictions or regions. It might do no harm to use one and the other in parentheses as a synonym for such places.
This reference is guidance on consent for clinical trials for adults who cannot make decisions themselves:

it uses the phrase 'legal representative'...

I'm assuming this trial document is for an incapacitated adult, because why else would they be sending information to a family member, rather than the patient????
