This is a difficust question. The "likuval'na sprava" specialization is a very general category. Basically, after receiving this general specialization the doctor has to study further and obtain more specific training - internal medicine, surgery, ophthalmology, obstetrics etc.

So what I would do is I would put

Medicine / General Medicine*

with a commentary:
*General Medicine here means that a further specialization is expected and therefore includes almost all fields of practical medicine, with the exeption of pediatric fields, which usually graduate with "Medicine / Pediatrics" degree.

You can also use some other options in the main text (e.g. "Medical Care", "Medical Practice"). I guess it wouldn't change much, it is the explanation that is important.
