In a personal medical record the physician enters the number of times a certain event has occurred. This is crucial information as it is paramount to the course of evaluation and therapy taken.

Visit number and Medical record number
In a hospital, each patient gets assigned a medical record number that follows them through all visits throughout their life. Each visit gets an identifying number as well. What this number is call can vary from hospital to hospital - incident, visit, identification, reference. I think visit is the most obvious and easily understood. I would definitely not call it an event number.
