My reading From my reading of it, the hand is cleft in the typical way (bilateral) with fingers missing in the middle. For this child, the ring finger and middle finger never grew (agenesia). In addition, this particular child has an extra thumb, and the two thumbs are webbed together. The surgery will close the cleft, excise the extra thumb, and do reconstructive surgery on the hand that now has three fingers.

Construction The more common term would be 'reconstruction', but here it seems there isn't enough basis for a "re"construction, hence my suggestion of 'construction'.

Cleft hand (Central Deficiency) reconstruction is a challenging surgical procedure. This child’s hand demonstrates some of the difficulties. There are 3 problems in the hand. First, the space between the thumb and the index finger is narrowed which limits his ability to grasp large objects.

1) Rearrange the soft tissues of the thumb- index web space. We use what is called a 4- flap z- plasty to deepen and improve the contour of the web space.

Aquí el término díficil es confección, pero he buscado algunos enlaces y veo que la palabra "confection" si es utilizada en el contexto de cirugía plástica. Así que por eso creo que ésta es la traducción mas apropiada.
