This is how I usually translate this term.

In your case, the GP (or family doctor) requested the X-ray.

You can also use requisitioner, but it's quite stilted, and not the usual term in practice, at least in the UK.

Requisitioner is not to be confused with the agency confiscating, expropriating or 'requisitioning' land and building.

Docs., purchase orders or copies can also be 'requested' on a requisition form or card that I recall from export practice.

Extraordinary etc. Company Meetings can be 'called' or requisitioned by a shareholder who is termed a 'requisitionist' - a comparatively little-known term of corporate art and often, inaccurately, turned on ProZ into a 'caller' of such gatherings: such party would rather, in the UK etc., be the Board of Directors or Company Secretary..
