It is followed by a date.
dt in next line ,which is again followed by adate is date o treatment.

not knowing what the medical examination is about, our suggestions are bound to be guess work only. let me suggest you consult the online dict.cc It offers about 5 or 6 possibilities, two of which are: differential diagnosis and diastolic dysfunction
