Are there English equivalents for these terms. I don't think guardian would fit.

These were recently much-(ab)used words in our family; an aunt was suffering from ALS (an advanced form of MS) and was almost helpless. She could have had an officially-appointed carer to look after her.

I think "Betreuung" in this case refers to the German legal situation where a person is appointed by a "Vormundschaftsgericht" (Guardianship Court) to manage someone else's financial and personal affairs. Part of a guardian's job may be to make medical decisions for the person whose affairs s/he is managing.

A Betreuer in this sense does not (necessarily) provide healthcare of any sort - it's about managing finances, making important decisions etc.
