HKT: (I assume this has something to do with insurance, as it is followed by the insurance company)

Because nothing is written after HA and EA, I can only make assumptions, and I doubt if HA means Hausarzt.

As this seems to be a block of administrative (rather than medical) information, I would read HKT = Hauptkostenträger, HA = Hausarzt, and EA = Einweisender Arzt.
