looks like the description here; similar to a person who has power of attorney in questions of health - usually one of the near relations who can be trusted to act on behalf of a relative who cannot take their own decision. May occur after an accident, for example. Someone without any near relatives (or perhaps, "relatives near" [e.g. in Australia?] may require a legally-appointed "carer".
