The information requested in this particular section is smoking (no. pack years), alcohol consumption, coffee, narcotics, risk factors.

I've never seen this information asked as a separate heading. In everyday conversation they would be "bad habits" or vices - any ideas for a neutral, clinical title? The best I can come up with is "dietary and other habits" - any ideas appreciated!
